Chronic low back pain like that explained by the veteran in this case report is very common and can be quite disabling. Spinal manipulation is endorsed for the management of chronic low back in many guidelines supported by such groups as the American Pain Society and the American College of Pain Physicians. (2,3) With chronic low back pain, central sensitization – the consequence of repeated nerve stimulation has on the central nervous system - is thought to have a role in chronicity of pain. A systematic review and meta-analysis of randomized controlled trials (47 RCTs and 9211 participants) of spinal manipulation care for chronic low back pain stated that spinal manipulation generated similar effects to other recommended therapies of chronic low back pain (like exercise and drug therapy), was better than non-recommended intervention for short-term improvement, and had limited, mild-to-moderate, passing adverse effects. (5) Another such meta-analysis determined that spinal manipulation appeared to be a good option for relief of pain and improvement of function for chronic low back pain sufferers. (6) In a meta-analysis specifically focused on older (ages 65-91 y/o) chronic low back pain sufferers, the same conclusions were found: spinal manipulation should be considered a positive treatment for the older patient with chronic low back pain for pain relief and function improvement. (7) Most spinal manipulation is done by chiropractors according to the National Center for Complementary and Integrative Health.
